Abstract
W niniejszym opracowaniu skoncentrowano się na znaczeniu oficjalnych dopłat ze strony pacjentów w finansowaniu świadczeń zdrowotnych. W pierwszej kolejności dokonano przeglądu różnych form współpłacenia stosowanych w wybranych krajach europejskich, a następnie przedstawiono korzyści i zagrożenia związane z zastosowaniem partycypacji pacjentów w kosztach świadczeń zdrowotnych we współczesnych systemach ochrony zdrowia. The paper deals with problems concerning the application of cost sharing (user charges) in the health care systems in the selected European countries. It is important to emphasize the increasing significance of private health care expenditure, including formal user charges, in the contemporary health systems, according to difficulties in funding health care by taxation or social insurance contributions. In practice user charges usually concern: Pharmaceuticals, dental care, selected ambulatory services and better conditions during hospitalization, however their level and the services to which they are applied vary considerable between particular European countries. The introducing or increasing user charges is expected to provide additional revenue for health care sector as well as to improve health system's efficiency. However, their implementation can also result in some negative effects, such as creating financial barriers in access to health care services and additional transaction costs. In the paper an overview of the main forms of user charges is presented and then the arguments for and against cost sharing are discussed. (original abstract)
