- Abstract
- Problem kosztów alternatywnych w ochronie zdrowia nabiera szczególnego znaczenia ze względu na znaczenie zdrowia dla każdego człowieka, dla społeczeństwa i dla gospodarki. Dokonanie wyboru między konkurującymi programami zdrowotnymi wymaga odwołania się do oceny ekonomicznej, czyli dokonania pomiarów wyników, do jakich programy te prowadzą. QALYs jest miernikiem uwzględniającym wartość zdrowia dla jednostki, czyli zmiany czasu trwania życia i jego jakości. Ze względu na znaczenie tego miernika przedstawiam jego konstrukcję, sposób jego obliczania, a także jego podstawy mikroekonomiczne. QALYs może być wykorzystany do skonstruowania optymalnego koszyka świadczeń medycznych, jak również do określania społecznego dobrobytu, co zostało opisane w kolejnych częściach. All resources have an opportunity cost and within health care there is a need to make decisions on the balance of resources. Comparisons among competing programs require some outcome measures that can incorporate quality and quantity of life changes. Such measures can be seen as measures of utility ("value of health") to individuals and are called cost-utility studies. Most of them measure quality-adjusted life years (QALYs). The concept of QALYs allows to make changes in the quality of life and changes of the length of life comparable. Because of very important role of QALYs in economic evaluation the article begins with calculating QALYs and then concentrates on microeconomic foundations of QALYs. In the next step the problem of constructing optimal basket of methods of treatment using QALYs is the point in question. After comparing cost-utility analysis and cost-benefit analysis it is possible to derive individuals' marginal willingness to pay for a QALY. Finally, analysis of social welfare is performed using QALYs.(original abstract) - Accessibility
